Permit Application Requirements

  • Completed building permit application
  • Signed Flatwork Requirements form
  • Legible copy of plat of survey showing location and dimensions of proposed work
  • Name of contractor
  • Cost of work
  • Permit fee based on scope of work
  • Zoning Permit Application

You will be required to draw on a copy of the plat, the shape, dimensions, and placement of the concrete that you are adding. A permit is required for all flat work, including but not limited to driveways, patios, sidewalks, garage floors, etc.

Severe Weather Restrictions

Due to weather conditions in our region, the Village of Orland Park has a cut off date for exterior concrete flat work. This period will begin on November 15th and extend to March 15 for flat work which typically includes driveways, sidewalks, and exposed slabs on grade. Refer to the Village Code Title 5, Chapter 1, Section 109.3.7 for inspection requirements and Section 109.3.7.1 for cold weather requirements.

Inspections Required

A pre-pour inspection of the base stone and forms is required. A final inspection is required once the work has been completed. A 24-hour notice is required for all inspections. View the concrete inspection request form.
